| Methadone: Risk Groups

Pregnant
women: Methadone is less problematic than illegal drugs. Nevertheless, the impact on the child is considerable: withdrawal syndrome in the neonate; long-lasting effects cannot be excluded.

Nursing
mothers: Methadone is excreted through breast milk; doses of up to 20 mg/day do not seem to cause any problems. Effects are possible if doses are higher. |
|
Children:
Increased risk of respiratory depression in infants. Childrens' dose for severe pain: 0.2 mg/kg every 6 hours.
Adult doses are life-threatening.

|
Elderly
people: Begin with the smallest doses; increased risk of respiratory depression. |
|
Renal
failure: Reduce dose by 50% or increase interval when there is advanced failure.
|
|
Liver
insufficiency: Half-life
is only prolonged if there is severe cirrhosis: reduce dose!
